An affiliated local training club must be formed with at least four members of Service Dogs of America whom will be the club's president, vice president, secretary and treasurer. Annual affiliation club dues are $150.00. After its first year, an SDA affiliated club is expected to put on at least one trial per year unless extenuating circumstances moves the SDA Board to excuse the trial requirement on a year by year basis. Only SDA affiliated clubs may host a trial offering the full range of UKC/SDA Dog-Sport titles. Individual SDA members are not required to belong to an affiliated club.

The paperwork for a local club to become an SDA affiliated club is as follows:

(5) List of club members including designation of club officers and

(6) Copy of your club's individual club member's application form completed

      to become a member of your local club (optional).

(6) Payment of the $150.00 annual affiliation dues (required).
